Cheekbones and midface
When the cheekbone loses volume, the cheek descends and the fold becomes more pronounced. Restoring support here often softens the fold without treating it directly.

The nasolabial fold is the crease that runs from the nose to the mouth, and marionette lines run down from the corners of the mouth toward the chin. They become more pronounced when the cheekbone loses volume and the skin loses collagen. That's why the best result almost never comes from filling the line, but from restoring support where it was lost.
At her practice in El Poblado, Dr. Olga Gómez Montoya, physician and surgeon from the University of Antioquia, studies where the fold comes from before proposing a plan. The goal is for you to look rested and fresh, without an overfilled face.

It is the crease that runs from the side of the nose to the corner of the mouth. Everyone has one: it is part of the face's anatomy. What changes over the years is how pronounced it looks.
They are the lines that run down from the corners of the mouth toward the chin. They can give a sad, serious or tired look even when you're not.

The fold and marionette lines are the consequence; the cause often lies in the cheekbone, the jaw or skin quality. That's why the plan may include areas you didn't expect.

The crease that runs from the nose to the corner of the mouth. It is softened with judgment, without flattening it: it is a natural crease of the face.

When the jawline loses definition, marionette lines become more noticeable. Supporting the jaw helps hold up the area.

A well-supported chin balances the lower face and softens the transition toward the marionette lines.
Thin skin or skin with less collagen makes creases more visible. Biostimulators improve its firmness and density.
First, support is restored where it was lost and then, only if needed, the line itself is treated. That way the result looks natural, not filled. Depending on your case, we combine:
Provides support in the cheekbones, jaw and chin, and softens the fold or marionette lines in conservative amounts. The change is visible from the session and settles in about 15 days.
They stimulate your own collagen to improve skin firmness and density. The change is progressive, over 2 to 3 months, and usually lasts around two years.
They provide support to tissues that have descended and stimulate collagen as they are absorbed.
Stimulates collagen in deep layers to improve firmness of the lower face, without needles.
In minimal doses, it relaxes the muscle that pulls the corners of the mouth downward and softens marionette lines.
“True aesthetic changes aren't seen; they're felt. In the naturalness of the result.” — Dr. Olga Gómez Montoya, physician and surgeon, University of Antioquia
When the fold is filled directly and in large amounts, the area looks heavy and the face loses its naturalness. In contrast, when support is restored in the cheekbone or jaw, the fold softens on its own and your expression is preserved.
This is common and doesn't always need treatment. It may be due to face shape, genetics, weight loss or a strong facial expression. Sometimes providing support in the cheekbone is enough; the assessment determines whether it's the right time.
We use only INVIMA-registered products (INVIMA is Colombia's health products regulator), in conservative amounts and with a technique adapted to the anatomy of the area. If your case involves marked laxity, we honestly refer you to a non-surgical facelift or a surgical assessment. Also learn about facial harmonization and collagen biostimulators.
The doctor analyzes your face from the front, in profile and in motion to determine whether the fold comes from the cheekbone, the skin or both, and takes clinical photos.
She defines which areas to treat first, which product and how much. We show you each product and its INVIMA registration. Less is more.
With topical anesthesia, using a microcannula or needle depending on the area. You return to your day with clear instructions for the first few hours.
At 15 and 30 days we review the result and adjust only if needed. Afterwards we define maintenance.

Mainly because the cheekbone loses volume and the cheek descends. Collagen loss, menopause, weight changes, sun, facial expression and genetics also play a role. That's why treating only the line is usually not enough.
Not completely, and it's not advisable: it is a natural crease of the face and flattening it looks artificial. What can be achieved is to visibly soften it by restoring support where it was lost.
Support is usually provided in the cheekbone first and, if needed, the fold is softened with a small amount, using a microcannula or needle. We use INVIMA-registered brands such as Juvéderm®, Restylane®, Stylage® or Croma®, and the technique is adapted to your anatomy.
Yes. In younger people it may be due to face shape, genetics, weight loss or a strong facial expression. Often it doesn't need treatment, or providing support in the cheekbone is enough. The assessment determines this.
They don't compete. Hyaluronic acid provides support and softens from the session onward; biostimulators such as Sculptra® or Radiesse® progressively improve skin firmness. They are often combined depending on the cause.
It depends on the product, the area and your metabolism. Biostimulators usually last around two years. At your check-up we tell you when maintenance is advisable in your case.
The most common are mild swelling or temporary bruising. Less often, nodules or asymmetries may appear. Vascular complications are uncommon but serious, which is why the procedure is always performed by a physician who knows the anatomy of the area.
